I think the problem is coming back to Canada. The custom will stamp your passport or put in a record in the computer system. I don't see any mark on my passport when I left Canada.

你填入籍申请表的时候,要附上护照复印件.这时,你不用每页复印.但是当你考笔试那天,他们会在考试前仔细检查你的所需证件,包括护照,这时他们会逐页检查你的护照.

I didn't travel by B1/B2, no record for existing/valid US visa, like F-1, H-1. etc.
